Healthcare organizations operate within strict privacy regulations and often have fragmented patient data across EMRs, billing systems, CRM platforms, and digital channels. We implemented a Salesforce-powered solution combining Salesforce Data Cloud, Marketing Cloud Engagement, Marketing Cloud Personalization, Einstein AI, Health Cloud, and Salesforce Shield to unify patient data, automate personalized journeys, and deliver relevant healthcare experiences while maintaining privacy and compliance.
The healthcare organizations wanted to:
1. Data Cloud for Unified Patient Profiles
We integrated patient data from CRM systems, Electronic Health Records, billing platforms, and mobile applications into Salesforce Data Cloud. This eliminated data silos and created holistic patient profiles containing demographics, diagnosis codes, and engagement history while respecting privacy through field-level encryption and role-based access.
2. Segmentation and Predictive Analytics
Using Data Cloud segmentation and predictive capabilities, patients and HCPs were grouped based on specialty, conditions, and behavior. Einstein AI provided insights into the likelihood of medication adherence and follow-up requirements. These segments were then activated in Marketing Cloud Engagement to trigger relevant journeys.
3. Marketing Cloud Engagement Journeys
We built automated onboarding journeys that welcomed new members and guided them through benefits and digital tools. Educational journeys provided information about coverage topics such as out-of-pocket costs and drug formularies using drip emails and push notifications.
Chronic-care support journeys delivered resources related to remote monitoring tools and peer support. For healthcare providers, location-based appointment-booking journeys and post-urgent-care follow-ups were created to educate patients about lower-cost care options.
4. Marketing Cloud Personalization
Real-time personalization was activated across web and email experiences. Data Cloud unified profiles and segments were passed into Marketing Cloud Personalization to deliver content based on recent interactions.
Patients who had recently received a diagnosis could receive tailored educational resources and medication reminders. HCP-facing portals could display dynamic content such as clinical trial updates and CME course recommendations based on specialty and previous interactions.
5. Privacy Controls and Compliance
Throughout the solution, Health Cloud and Salesforce Shield were configured to support encryption, audit trails, and event monitoring. Preference centers allowed subscribers to manage their opt-in levels, while de-identified data was used for segmentation where explicit consent was not available.
